Aims

The aim is to help managers develop a systematic approach to the investigation and analysis of out of specification events that will target root causes and allow purposeful corrective and preventive action to be applied

Objectives

At the end of the course each delegate should be able to

Explain the relationship between injury, damage and near miss events
Apply appropriate resources to the investigation of unwanted events
Use the domino theory to identify the sequence of cause and consequences in out of specification events and develop causal trees
Analyse information to identify root causes
Select and apply effective technical, procedural and behavioural solutions to problems identified by the analysis
